Which Mail Service Pharmacy should I use?

ClearScript’s prescription delivery service is provided by Fairview Mail Service Pharmacy. Alternatively, if your employer owns/operates a mail service pharmacy, please check your plan summary documents for additional information or directions for use.

How do I start using the Fairview Mail Service Pharmacy?

By Phone: To begin having your prescriptions delivered to your home or other location, call Fairview Mail Service Pharmacy at 1-866-377-6245 (toll-free).

By mail: Print out a mail service application form. Complete the information required and mail it with your doctor’s prescription enclosed.

If you need to start your medication right away, please have your physician complete two prescriptions. One prescription needs to be written for a one-month supply to bring to your local pharmacy. Mail the second prescription along with the application form. It is helpful for the mailed prescription to be written for a 3-month supply.

How do I transfer a prescription from another pharmacy?

Call Fairview Mail Service at 1-866-377-6245. They will gather the required information and complete the transfer on your behalf.

How do I get additional refills through the Fairview Mail Service Pharmacy?

By Mail: Mail back the pre-printed order form that comes with each shipment.

By phone: Call 1-866-377-6245.

Online: Go to MyFairviewRx.org, select ‘Refill by typing Rx number’ and choose ‘Mail order’ (not ‘In-store’) for your pick up preference. Or, register for an account to get additional features such as automated refill reminders.

Mobile App: Get the MyFairviewRX mobile app available at the Apple App Store or Google Play.

NOTE: Each family member requires a separate email address to log in.

How is confidentiality ensured?

Your medication will be sent to you in a plain package with a return address only. The pharmacy name will not be listed on the package.

How do I pay for the service?

Co-payments may be made by credit card. Credit card information must accompany your prescription request.

How long will it take to receive medications through the mail service?

You can expect to receive your medications 3 to 5 days from the time Fairview Mail Service Pharmacy receives your valid prescription order. If you need your medication sooner than this time frame, please call the mail service pharmacy to discuss your options and make new arrangements if necessary.

Is there a charge for postage?

For regular mail delivery, there is no charge for postage. For requested next day service or special handling, there will be an additional charge.

Can I talk to a pharmacist?

Yes, call 1-866-377-6245. Pharmacists are available Monday through Friday, 8 a.m. to 7 p.m. and Saturday, 8 a.m. to 4 p.m. (CST).

How do I access my medications if there is a delay, I have an emergency situation, or a disaster occurs?

If there is ever a question about your order, or what to do about your care in any situation, please contact the pharmacy and they will assist you. They will work with you and your care team to develop a plan to ensure you have access to your medications.

How do I find out about the status of my order, including delays?

If there is ever a question about your order, or what to do about your care in any situation, please contact the pharmacy and they will assist you. If they know a delay with your order will occur, they will contact you to discuss the delay and make new arrangements if necessary.
