ClearScript’s prescription delivery service is provided by Fairview Mail Service Pharmacy.
To begin having your prescriptions delivered to your home or other location, simply call Fairview Mail Service Pharmacy at 1-866-377-6245.
Or, you can print out a mail service application form, complete the information required, and mail it with your doctor's prescription enclosed.
Once you have started mail service, you can request refills online, by phone, or using the pre-printed order form that comes with each shipment.
